If you would like to report an instance of misconduct or maltreatment, you may do so on this form. You can submit a report as a victim or witness of misconduct or maltreatment.

If you would like to submit the report anonymously, you can omit names and ignore the question where we ask for your contact information. However, please note that submitting a report anonymously may make it difficult for us as a club to fully address your concerns. Nonetheless, we respect your privacy.
